



Expanded In-home Services for the Elderly Program
Expanded
In-home Services for the Elderly Program (EISEP) assists older adults who
need minimal assistance with housekeeping and/or personal care and non-institutional
respite services through various social adult day programs. EISEP services
are intended for long term, non-medical situations. EISEP also matches each
client with a case manager who helps determine what help is needed and provides
on-going guidance to the client and family. Housekeeping services may include
light housekeeping, laundry and/or meal preparation.
Personal care may include bathing, dressing, grooming and/or ambulating. Non-institutional
respite care is offered through subcontracts with five social adult day programs
throughout the County. Case management includes an in-home assessment, links
to appropriate services and monitoring of services. Jewish Family Service
of Rochester, Inc. provides case management services directly and also collaborates
with the following agencies for case management services:

ELIGIBILITY CRITERIA: You must be a Monroe county
resident, at least 60 years of age and need assistance with activities of
daily living. If you are eligible to receive similar services under Medicare,
Medicaid or any other government program, you are not eligible for EISEP.
FEES: There may be a modest fee for the home care and/or respite services,
depending on your monthly income. The case management portion of the program
is provided free of charge but donations are accepted.
PROGRAM LOCATIONS: This is a home-based program.INTAKE PROCESS: Call
(585) 262-7060 for an appointment. Basic information is taken over the phone
and a case manager is assigned to conduct an assessment in the home and develop
a care plan. Services are then arranged based on the care plan.

Potential clients must have at least one unmet activity of daily living (ADL)
and two unmet instrumental activities of daily living (IADL) to qualify.
Project 2015 is examining the impact of the aging of the Baby Boom generation
in New York State. It includes briefs by State agencies, articles on aging
topics, selected population characteristics of New York State counties and
projections of future demographics.
