Home Base is a community partnership that promotes independent, safe and healthy environments for aging adults living in their own homes. The program helps meet the changing needs of older adults living at Ellison Park Apartments, where JFS Home Base staff are located . The Home Base social workers connect residents with programs and serices that support successful aging in place in their own apartments.

The partners of the program include the residents, community service providers and the property management company.

Home Base Services

Home Base services are free of charge to adults over 60 years of age living at Ellison Park Aparments in Rochester, NY.

• Transportation for grocery shopping, social/recreational/exercise activities and other destinations

• Scheduled in-home assistance with laundry, housekeeping and daily living activities

• Referrals to in-home medical care, Personal Early Response Systems (PERS) and friendly visitor programs

• Comprehensice care management and case assistance

• Monthly resident meetings designed to enhance socialization and plan events

• Computer classes

• Inter-generational programs and social events

• Educational and health related workshops

• Counseling services and caregiver support
