Home Base is a community partnership that promotes independent, safe and healthy environments for aging adults living in their own homes. The program helps meet the changing needs of older adults living at Ellison Park and Eastwood Apartments, where JFS Home Base staff are located . The Home Base social workers connect residents with programs and serices that support successful aging in place in their own apartments.

The partners of the program include the residents, community service providers and the property management company.
